Form+ for Google Forms for schools

A school can give Form+ for Google Forms to staff and students in bulk through Apple School Manager, and the app signs in with the school's own Google Workspace for Education accounts. One setting in the Google Admin console decides whether students under 18 can sign in. This page is for the two people who set that up: the person who manages the school's Apple devices, and the Google Workspace administrator.
Getting the app onto school iPads and iPhones
Form+ for Google Forms is free on the App Store (App Store ID 1544827472). In Apple School Manager it is found by name. Apple's steps, from the Apple School Manager User Guide (Select and buy content):
“Select Apps and Books in the sidebar, then select or search for an app or book in the search field.”
“Enter the number of licenses, and if necessary, change the payment method, then select Buy.”
“Note: You need to set up your payment method before you can obtain app licenses, even if they’re free.”
Licences are then assigned to devices from your device management system, as for any other app.
Signing in with school Google accounts
Staff sign in with their school Google account as they would on the web. For students, Google has a rule that catches many schools out. From Google Workspace Admin Help (Manage access to unconfigured third-party apps for users designated as under 18):
“Users designated as under 18 by the age-based access setting are blocked from using unconfigured third-party apps.”
“An unconfigured third-party app is any app that isn't yet configured with an access setting (trusted, limited, or blocked) in the Admin console.”
So if students see a “blocked” message when they sign in, the app has not been configured for your domain yet. Google's page describes how an administrator configures an app; the detail you will be asked for is the app's OAuth client ID:

Google also notes: “When a user designated as under 18 tries to access an unconfigured app, they'll be blocked, but see a message with the option to request access.”
What the app asks Google for, and why
- Google Drive — to list, open and edit the forms in the signed-in account, and the spreadsheets their responses go to.
- Apps Script (forms, e-mail, triggers, external requests) — for the automation tools: response limits, opening and closing a form on a schedule, and e-mail notifications.
- The account's e-mail address — to show which account is signed in.
Where the data lives
Forms and responses stay in the school's own Google account and Google Drive. Form+ works on top of that account rather than copying forms and responses onto a separate server. The optional AI tools are the exception: when someone uses them, the question and the answers being analysed are sent through our server to an AI provider to be processed. Nothing is sent unless those tools are used. The full policy is the Privacy Policy.
